Symptom narrative

No adverse effects reported; female inadvertently received an expired (supported via post expiry memo) dose of MMR II; due to system error with their scanning workflow; This spontaneous report was received from a nurse concerning a 12-months-old female patient. Her concurrent conditions, medical history or concomitant therapies were not reported. On 02-DEC-2025, the patient was inadvertently vaccinated with an expired dose of Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R-II), vial, (lot number: X027447, expiration date: 27-NOV-2025) administered as single dose as prophylaxis (route or site of administration were not provided). The error occurred due to system error with their scanning workflow (circumstance or information capable of leading to medication error). No adverse events were reported.
