On 01-DEC-2025, the patient was inadvertently vaccinated with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) Injection, administered as prophylaxis (lot number reported as Z008509, which has been verified as valid, expiration date reported and validated as 23-JUL-2027) (strength, dose, route of administration, and anatomical site of injection were not reported). The vaccine was administered inadvertently after the shipment arrived late. The temperature was unknown. The received by date on the packing slip read 29-NOV-2025 but it did not arrive until 01-DEC-2025 (Product storage error). No additional adverse event reported.
