Symptom narrative

I work at a hospital. It is required that I that I get the flu vaccine. I went to the clinic to get the flu vaccine but was given the covid vaccine instead. I am in the first trimester of my pregnancy, so I do not have a due date at this time. My left arm is very sore and I have lower back but no other symptoms. I want to have this documented because I was given the wrong vaccine. I had the covid vaccines in the past back in 2021 and 2022 at Pharmacy.
