The patient's concurrent conditions, medical history, previous drug reactions, drug allergies and concomitant therapies were not reported. On 01-Dec-2025, the patient was vaccinated with an improperly stored dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) 0.5 mL Injection (involved lot # was Z008197 which has been verified to be valid, expiration date: 30-Apr-2027), reconstituted in a sterile diluent and administered as prophylaxis (route of administration and site of administration were not provided). Temperature excursion was -10.5C for 1 hour 0 minutes 0 seconds. There was no previous temperature excursion nor report of adverse effect or product quality complaint involved.
