Reported Symptoms: 10012703:DIAPHORESIS; 10013573:DIZZINESS; 10042772:SYNCOPE; Narrative: Patient lost consciousness in clinic. A rapid response was called and patient was transported to the Emergency Department for workup. Other Relevant HX: during clinic visit with gastro, patient received his annual influenza vaccination. Approximately 5 minutes after receiving vaccination, patient reported he felt dizzy. Patient became diaphoretic. Vitals were checked. BP was 108/74 ; HR 74; SPO2 95%; Blood glucose 47. Patient is not a diabetic. He denied eating this morning. Skin became pale, cool and clammy. Patient reported his vision was becoming tunnel visioned. Rapid Response called. Patient then went limp and lost consciousness. Code Blue was called. Patient was laid back and he regained consciousness. Report was provided to the code team successfully and then transferred to the ED. REASON FOR VISIT (Chief Complaint): Rapid response due to seizure-like activity but then stated that he was fine immediately when he woke up with no lethargy or other postictal symptoms. The patient denies having any chest pain or shortness of breath or nausea or vomiting. He does note that 2 years ago when he received a COVID-vaccine he became very sweaty and felt like he was going to pass out.
