Reported Symptoms: 10005886:BLURRED VISION; 1001033:CONFUSION; 10012703:DIAPHORESIS; 10013573; DIZZINESS; 10016379: FELT FAINT; 10024490:LIGHT-HEADED; Narrative: A rapid response was called and patient was transported to the ED. Other relevant HX: During GI appointment patient reported sudden blurred vision and lightheadedness. Patient received his flu shot at this appointment. Patient states he has experienced this before and denied ER treatment and stated it would go away in a little while. Once Rapid response team left, the patient agreed to an evaluation due to his allergy history and just recently getting the influenza vaccine today. REASON FOR ED VISIT (Chief Compliant): Visual disturbance Patient presents to the emergency department after experiencing visual disturbance and a specialty clinic. He says this has been going on for the last 50 years. He says twice a week he feels like there are dragonfly wings in his vision. He says it lasts anywhere from 20 minutes to an hour and then resolves. He says over the past 50 years he has multiple scans and workups and no one can seem to tell him why this happens. He says that since his cataract surgery he has had 20/20 vision with distance and uses glasses for reading. Currently in the ED he had no visual disturbance and says that he can see well. He denies any slurred speech, numbness, tingling or weakness. The patient denies any suicidal ideation to me. MDM/ED treatment: We were prepared to do a full workup on patient in the ED.
