Symptom narrative

no additional AE; PROQUAD was administered after being involved in a temperature excursion; This spontaneous report was received from a certified medical assistant and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 13-Aug-2025, the patient was vaccinated with a dose of improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), Solution for injection (lot #Z002989, expiration date: 14-Jul-2026) (1 dose) reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), Solution for injection (indication, expiration date, and lot # were not reported). The vaccine was involved in a temperature excursion of 21.9 F, 14 F to 46 F for 12 hours 30 minutes and 6 F to 14 F for 37 hours. There was no previous temperature excursion. No additional adverse event was reported.
