Symptom narrative

Pt came in 10/7/25 for a well visit and was due for their Meningococcal and Men B vaccines. Pt had also opted to receive their flu vaccine. When it was time to administer the vaccines, I had accidently given the pt a Hep B vaccine instead of the Men B. When I realized what I had done, I immediately informed the provider and then informed the pt. The provider said there was no harm done, and f anything the pt would have extra immunity. I also informed pt of that as well. I apologized several times to pt and she said, ?it?s okay.? I asked if they still wanted to receive the Men B vaccines, or did they want to wait and come back in. Pt stated she wanted to receive the vaccine before she left. I then administered Men b vaccine.
