Symptom narrative

Patient received first dose on 7/12/2024 and second dose 6/27/2025, patient wanted a third dose since 11 months between dose administered. Patient did not return to pharmacy since was attending school. Pharmacist on 6/27/2025 administer dose and instructed patient to return in 6 months, not aware of the 7/12/2024 dose. Pharmacist on duty 12/29/2025 instructed patient was complete since Health Shots indicated series complete and give patient a copy of the form. Pharmacist on 12/29/2025 did not administer a third dose to patient since received the 2 dose series and verified with the mfg.
