Symptom narrative

Patient experienced muscle atrophy and deterioration at the injection site. The patient has missing muscle where the injection was administered. The patient had a notable indent in the tissue about 3 inch long by 3 inch wide where no muscle tissue is present. The patient reports muscle weakness in the right arm that the vaccine was administered. Patient sought out muscle testing that was positive for abnormality at the chiropractor. Patient noticed symptoms at holiday..

Medical history

allergic rhinitis, mixed anxiety and depression, on depo provera contraception

Other medications

escitalopram 10 mg tab - take 1 by mouth daily, loratidine 5 mg/5 ml oral syrup - take 10 ml daily, montelukast 10 mg tab - take 1 tab by mouth every evening, depo provera 150 mg - inject 150 mg intramuscular every 3 months.
