Time:10:00 am Pulled vial out of MMR box in refrigerator and administered it in RT/SC at 10:00, upon entering lot noticed it was not MMR and was MMRV. Informed provider immediately. Provider notified parent. Parent was advised @ 10;20 am, encouraged to contact our office if concern arose. Child left office. MMRV administration notified and spoke with employee at 11:53 am, was informed to revaccinate as MMRV invalid not frozen was refrigerated and was informed to report to VAERS.
