Site: Pain at Injection Site-Mild, Additional Details: No adverse reaction occured. Inadvertly second dose of flu vaccine was administered on 12/20/25. Intial flu vaccine was given in 10/20/2025. Reason was she have a twin brother by the name of [withheld] with same date of birth. when patient came in I checked in twin for vaccines and administered flu and covid. But she came in only for covid vaccine.I told her to notify her prescriber as well .
