Symptom narrative

Systemic: Chills-Severe, Systemic: Confusion-Severe, Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Medium, Systemic: Nausea-Medium, Systemic: Numbness (specify: facial area, extremities)-Severe, Systemic: Seizure-Severe, Systemic: Shakiness-Severe, Additional Details: Patient received Flaurix, Hep B, and TDAP on Left Arm within minutes of each other. After administration of last vaccine(Heplisav) patient went into convulsions, shakes, chills, confused, nausea. EMS was called and patient was stabilized and released. Patient states she hadnt had any shots in over 20 years, no HX of seizure eplilepsy.
