Symptom narrative

Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Nausea-Mild, Additional Details: Per patient's father, patient complained of dizziness approximately 5 minutes after vaccine while walking around store then fainted. She was caught by her father and no injury was reported. Patient quickly regained consciousness per witnesses. At this point, patient appeared ashen and reported nausea. Patient was seated with feet up and reported full recovery approximately 10 to 15 minutes later and left under her own power with her father. Medical attention declined.
