Symptom narrative

Pateint received two doses of influenza vaccine within the same season, approximately 3 months apart; 1st dose on 9/26/25 and 2nd dose on 12/23/25. Patient was monitored and had no reaction from influenza vaccine. Patient was informed of this event and the flu vaccination is usually once a year for her age, disscussed possible side effects of influenza vaccination, and instructed if any problems with vaccinations to follow up with urgent care or emergency department. Patient verbalized understadning.
