Symptom narrative

1) After this vaccine I developed Afib on 10/29/2021. Original injection was at the same location on 3/2/2021. I also had a second injection on 3/21/2021 at the same location.

Lab data

EKG. Unsuccessful Cardio-version completed on 12/31/2021. Successful Cardio Ablation completed on 4/1/2022
