No additional AE; PROQUAD was administered after being involved in a temperature excursion; This Spontaneous report was received from a Medical Assistant and referred to a patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant medications were not reported. On 21-Aug-2025, the patient was vaccinated with an improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) 1 dose, Lot Number: Z002989 and Expiration date: 14-Jul-202 (route and anatomical site of administration were not reported). The vaccine was reconstituted with a sterile diluent (Lot# and expiration date not reported) as prophylaxis. The vaccine had a temperature excursion of 21.9F. Time frame: 14°F to 46°F for 12 Hours 30 Minutes 0 Seconds; 6°F to 14°F for 37 Hours 30 Minutes 0 Seconds. There was no previous temperature excursion. No additional Adverse event reported.
