Symptom narrative

Patient fainted and fell forward while getting up from the exam chair after receiving the Menveo Vaccine. Mother of patient stated he did not eat or drink before the appointment. Patient's mother also stated that patient has high anxiety about shots. Patient was alert and oriented during the visit. After the faint and fall, patient asked questions about what happened and was able to ask questions and follow directions given by staff and doctor. Vitals were taken and within normal limits. Patient had redness on left cheekbone and head. Patient was given a juice box and ice pack and discharged home by his doctor.
