Symptom narrative

Patient states he is experiencing left sided shoulder pain which began 2 days after receiving his flu vaccine. Pain has not improved and progressively become worse. Patient states he is having decreased range of motion in his shoulder. Saw an orthopedics provider who suggested he may have SIRVA (shoulder injury related to vaccine administration). He is scheduled to see physical therapy for ongoing concerns.

Xray of left shoulder performed on 12/19/25 which revealed normal glenohumeral joint. Minimal osteoarthritis of the AC joint. No other significant abnormality.
