Symptom narrative

The medical assistant opened a new box of MMR vaccines and noted one vial of vaccine did not have a cap intact covering the top of the vial to protect the puncture pad. Noted the vial behind the uncapped vial had 2 caps on it. Medical assistant notified RN that cap was missing on vial. RN noted a small circular mark on puncture pad. RN infirmed medical assistant to not use and to get a new vial that has a safety cap intact. lot number Y018962, exp 1/14/2026
