Symptom narrative

Describe Event, Problem, or Product Use Error: 78 y.o. female with relevant PMHx of PH, HFpEF, complex mixed sleep apnea, persistent Afib, and gout, presenting with severe, disseminated rash involving the oral mucosa that started on 11/1/25. Rash that began on 11/1 is most consistent with DRESS (RegiSCAR 5) with inciting drug likely allopurinol started on 9/15. Patient has been continuing to take allopurinol. Has elevated bilirubin at baseline but appears to have uptrended. Eosinophilia present. Eruptions possible with flu vaccination, and cases of DRESS (however flu vaccine administered 48 hours prior to eruption). There are cases of patients on both allopurinol and having received recent flu vaccination where the combination potentially triggered DRESS. DRESS syndrome potentially induced by allopurinol and triggered by influenza vaccine. Patient admitted for DRESS treatment on 11/16.
