Symptom narrative

Patient scheduled an online appointment for XX/XX/2025 with XXXXXX Pharmacy to receive a covid vaccine. Upon presenting to the pharmacy intake counter, patient verbally confirmed with the pharmacist that they would be administering the covid vaccine. After patient received the vaccination, no receipts or proof of vaccination was provided to the patient. Patient departed the pharmacy believing they had received the covid vaccine. Later it was discovered that the flublok vaccine was administered to the patient instead of the covid vaccine. This patient had already received the flu vaccine for the 2025-2026 season in September 2025, therefore the pharmacy mistakenly duplicated an unnecessary vaccination. VAERS report has been filed as well as a complain through pharmacy. Patient was reviewing insurance claims and discovered the claim for the flu vaccine on the date of service, XX/XX/2025, without any claims for the covid vaccine. Patient received records with the state immunization registry and the pharmacy that administered the vaccine. Pharmacist should have verbally confirmed which vaccine they were administering to the patient immediately prior to injection, and also shown the vaccine vial or syringe to the patient for visual inspection. Patient should have been provided with a receipt of vaccine administration.
