Symptom narrative

Patient received IM Influenza injection to R Deltoid, initially tolerated well. Approximately 5 minutes after injection while seated in chair patient became pale and had brief LOC lasting approximately 5-10 seconds. Subsequently reports that he had a brief dream but was still feeling "fuzzy" with blurred vision. Became diaphoretic. Did not want to lay on floor. Ice packs placed under arm pits and on forehead. Had one episode of emesis. Remained awake and alert. Vitals taken 2 x with stabilization. Patient was given apple juice and nutrition bar. Patient remained seated for approximately 30 minutes with good resolution of all symptoms. Patient was able to stand and ambulate while remaining asymptomatic. Patient was then discharged to home with mom and sister. Site of injection with no redness or swelling. No further symptoms reported
