Symptom narrative

Patient reported side effects to vaccine at following office visit on 12/22/2025. This was nor reported at the time of incident. Pt reported shortly after vaccine she had symptoms of feeling "off", shortness of breath and anxiety which lasted about 20 minutes and then felt "normal"
