Symptom narrative

Patient stated the arm in which the vaccination was given was more painful than anticipated following vaccination. He states he had pain and issues with rotating the arm. He believes this is attributed to the immunization. The concerns rotating the arm is still present 2½ months following vaccination. Medical provider visits have been completed to evaluate and follow up are scheduled in the next few weeks as well.
