A 9-year-old female patient received BNT162b2 omicron (lp.8.1) (COMIRNATY (2025-2026 FORMULA)), as dose number unknown, single (Batch/Lot number: unknown) intramuscular for covid-19 immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: POOR QUALITY PRODUCT ADMINISTERED (non-serious), outcome "unknown", described as "used before November 26th and would like to know the stability because they administered one to the patient"; PRODUCT ADMINISTRATION ERROR (non-serious), outcome "unknown", described as "received a box of the vaccine from wholesaler which had a put the 10 week refrigerated as 26Nov2025". The information on the batch/lot number for BNT162b2 omicron (lp.8.1) will be requested and submitted if and when received. Additional information: Pharmacist called about the Covid Vaccine 5-11 years and would like to get storage information because they received the vaccine today and the wholesaler said it must be used before November 26th and would like to know the stability because they administered one to the patient. Confirmed caller information and inquiry, caller stated they received a box of the vaccine from wholesaler which had a put the 10 week refrigerated as 26Nov2025, caller want to see if Pfizer have information if they need to revaccinate and if the vaccine is stable at refrigerator temperature. Inquired if they confirmed with wholesaler if the beyond use date is 26Nov2025, caller said they called and the wholesaler cannot help them. Confirmed and inquired if vaccine was administered, vaccine was given to a 9-year-old, female.
