An 82-year-old male patient received BNT162b2 omicron (lp.8.1) (COMIRNATY (2025-2026 FORMULA)), on 24Nov2025 as dose 1, single (Batch/Lot number: unknown), in left arm for covid-19 immunisation. The patient's relevant medical history included: "heart attack" (unspecified if ongoing), notes: goes to his heart doctor a couple times a year; "had stents" (unspecified if ongoing). Concomitant medication(s) included: INFLUENZA VACCINE taken for immunisation, on 24Nov2025. He had Flu shot on his right arm and COVID shot on his left arm and over the years, he had never had any reactions. The patient has had probably 8 or 10 COVID shots over the years, always Pfizer, and he never had any reaction from a vaccine, he did not even have the slightest sore arm where he got the shot, he never had any reaction at all. But for this he had a slight sensitivity, that he had until now. He also felt tenderness. But for the last 2 to 3 days it's gotten more sensitive on his left arm. He is not in pain, only when he squeezes it, he feels muscle soreness or tenderness when he brushes off his left arm. He hasn't had any severe reaction that stopped him on doing anything and the pain at the injection site is almost a month and now it seems to be a little more prevalent, it's been 4 weeks now. He stated that it's not anything that feels serious. He further explained that after the COVID shot in his left arm, he had a slight feeling like he had gotten a shot, nothing really ever to be concerned with and he almost has to even touch it, to even feel it, and so he did not think anything other than overnight it could go away and it seemed to go away after a few days or he thought it was going away. Now, he is a side sleep on his right side and now, recently, in the last two days he is feeling that same like, feeling like he had a shot especially if he touches it, only a little bit more, still nothing that he would call really painful, just something that he would call annoying that he is aware of, and it almost seems like, underneath there is an area that if he kind of squeeze with his fingers, there is a little, he would not call it a lump, but a little bit of where the muscles seems a little, not quite as quite as pliable as in the other arm for almost as pliable, but if he touches it with my fingers, he can like, feel where he had the shot. Now, it is almost a month, there is no redness, there is no swelling, there is no discoloration, but he is curious because he has never had a reaction like this before and it is really not a serious reaction. It is just that he does have a feeling like he got a shot there and it was almost a month ago. No follow-up attempts are possible.
