Symptom narrative

pt says she received 3 vaccines on 12/19, 2 in right arm (capvaxive, flublok). in right arm she is complaining of weakness, unable to lift and no longer weight bearing, has pain spreading from area of vaccine to shoulder and into armpit, she also says she is extremely tired. pt used hand gestures and pointed to correct location if imz administration and gestured as pain went up to joint and down to armpit. based on hand gestures and questioning of patient, it seems vaccine was administered in correct arm location. I advised her to be seen by md and that I would turn in reactions to home office and to vaers.
