Injection site rash

Symptom narrative

Pt reported rash on right arm 1/2/26. Denies pain, itching, states she wouldn't have noticed it if she hadn't taken a shower today. about 2 inches below injection site, approx 3inX5in. Unclear if related to vaccine. Provider aware, did not think warranted abx. Pt states she did start using new body soap recently.
