Symptom narrative

Patient developed mild swelling and a red patch on right arm approximately 5 hours after vaccinations. Pain at injection site. Patient reports in office that pain is better today and redness is looking better today as well. Temps low 100s. Taking ibuprofen as needed. Can't sleep well. It is improving today, more faint redness. Not as tender. No firmness. Patient used a warm pack to the area. Redness on exam today measures 6 cm x 6 cm and is more faint on exam. No firmness. No tenderness.
