Symptom narrative

Patient presented to the pharmacy for his "2nd" dose of Shingrix on 01/03/2026. Upon reviewing the patient vaccine history I discovered that the patient had already been fully vaccinated with 2 doses of Shingrix on 09/05/2020 and 11/14/2020 and was not recommended to receive the vaccine that was administered on 10/16/2025. After this discovery I consulted with the patient who reported NO adverse events or side effects.
