Reported Symptoms: Narrative: 02/04/2020 Pt receives Shingles vaccine 02/12/2020 Pt c/o pressure pain behind eyes x 1 week & "cold" x 10 days 02/13/2020 Pt reports "was given the shingles inj on 02/04/20 and since then, has been feeling muscle aches, joint pain, pressure on the right side, behind the eye, headaches, OTC tx not effective, fever 100.8, has been coughing- light phlegm; initially attributed his sx to the shingles vaccine, could not get out bed. +trembling. +fever. +sweating. +sore throat, aching all over the body," suspected possible flu, treated w/ medrol dose pack, duoneb 02/14/2020 Pt tests positive for influenza A 05/13/2020 Seen by neurology "acute upper and lower extremity weakness - falling and imbalance numbness/weakness/paresthetic pain feet, arms, hands. one would consider acute idiopathic demyelinating polyneuropathy, guillaine-barre, as well as the possible side effects to the shingles vaccine and/or the possibility of the shingles or flu interaction, also must consider the possibility of possible abnormalities of the spinal cord, brain, vascular compromise.' advised IVIG, gabapentin" 06/12/2020 EMG results: moderate peripheral neuropathy of the bilateral lower extremity affecting the sensory and motor components of the nerves. the neuropathy is predominantly demyelinating in nature 06/26/2020 Pt reports "had flu after his shingles vacc; treated w/prednisone pak, after taking states fingers, legs & feet became numb, having increased urination, hospitalized x 7 days, readmitted & found pulmonary embolism" 07/24/2020 Seen by neurology "told likely has an acute
10/15/2021 Brain MRI No acute intracranial findings. Mild to moderate white matter T2 hyperintensities, grossly similar in extent. Mild parenchymal volume loss. 06/06/2022 MRI Lumbar Impression: 1. Persistent, severe, degenerative disc disease at L4-5 with degenerative changes both within the discs and facets within the 2 lowest lumbar disc space levels, as above, but without severe stenoses appreciated. There is moderate right neural foraminal stenosis at L5-S1. Please also see the above level by level descriptions. 2.
