Symptom narrative

Patient came to the pharmacy on 12/3/25 and presented with a softball sized skin reaction that was warm to the touch on the right deltoid. We reached out to her PCP for further examination. Patient's PCP determined that there was no infection at the injection site and that it was due to local reaction to vaccination.
