Symptom narrative

When Pt woke up from nap he was very fuzzy and inconsolable. He did have a red lump at vaccine site. Due to being inconsolable we advised pt to go to ER for evaluation. At ED pt was sleeping and the red lump had subsided. Seems to have been just a local reaction that only lasted a couple of hours. Pt was sent home and did not need to be treated or seen again for the local reaction.
