Patient was administered Kinrix instead of Pediarix. No symptoms or adverse outcomes. Incorrect vaccine was given. The prefilled syringe has the works all spelled out and our system has the abbreviations listed. Have asked the manufacturer if abbreviations can be added to prefilled syringe label
