Boostrix before recommended age / unintentional dose; Boostrix before recommended age / unintentional dose; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 15-month-old female patient who received DTPa (Reduced antigen) (Boostrix) for prophylaxis. Co-suspect products included DTPa (DTaP vaccine) for prophylaxis. On 29-DEC-2025, the patient received Boostrix. The patient did not receive the 4th dose of DTaP vaccine. On 29-DEC-2025, an unknown time after receiving Boostrix, the patient experienced wrong vaccine administered (Verbatim: Boostrix before recommended age / unintentional dose) and inappropriate age at vaccine administration (Verbatim: Boostrix before recommended age / unintentional dose). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. Additional Information: GSK Receipt Date: 30-DEC-2025 The medical assistant called, to report that, a patient who had been supposed to receive the DTaP vaccine had been accidentally given a dose of Boostrix. The medical assistant mentioned that the patient had been 15 months old and the intended vaccine would have been the fourth dose of DTaP for the patient. That accidental dose of Boostrix had been the first Tdap vaccine the patient had received. The batch number was not provided upon follow up with the reporter.
