On an unknown date, the patient received Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: 16 years old received the diluent portion only of Penmenvy, it was not mixed with the powder) and inappropriate dose of vaccine administered (Verbatim: 16 years old received the diluent portion only of Penmenvy, it was not mixed with the powder).
