A 53-year-old female patient received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), on 30Dec2025 as dose 1, single (Batch/Lot number: unknown) at the age of 53 , in left arm for immunisation. The patient's relevant medical history and concomitant medications were not reported. Vaccination history included: Shingles vaccine (dose number unknown, unknown manufacturer), for immunisation, reaction(s): "no issues except the sore arm"; Tetanus vaccine (dose number unknown, unknown manufacturer), for immunisation, reaction(s): "no issues except the sore arm". The following information was reported: PYREXIA (non-serious) with onset 31Dec2025, outcome "recovered", described as "fever"; CHILLS (non-serious) with onset 31Dec2025, outcome "recovered", described as "regular chills"; VACCINATION SITE ERYTHEMA (non-serious) with onset 03Jan2026, outcome "unknown", described as "red patches near the injection site"; RASH (non-serious) with onset 04Jan2026, outcome "recovering", described as "she has a rash/rash on the inside of her left arm"; DEHYDRATION (non-serious) with onset 05Jan2026, outcome "not recovered", described as "massive dehydration"; SCALLOPED TONGUE (non-serious) with onset 05Jan2026, outcome "unknown"; SPEECH DISORDER (non-serious) with onset 2026, outcome "unknown", described as "cant speak"; MALAISE (non-serious) with onset 2026, outcome "unknown", described as "she is feeling ill again"; NERVOUSNESS (non-serious) with onset 2026, outcome "unknown", described as "she's getting very nervous"; TONGUE DISCOMFORT (non-serious) with onset 2026, outcome "unknown", described as "tongue feels funny"; VACCINATION SITE PAIN (non-serious), outcome "not recovered", described as "injection site if still throbbing/Near the injection site it's pulsating again". Therapeutic measures were taken as a result of vaccination site pain, dehydration, pyrexia. Additional information: Patient wanted to know if there were variable doses of the Prevnar-20 for use in adult patients and if Pfizer has "increased something in the dose" for adult patients since only one vaccination per lifetime is recommended. She stated she has been having side effects after receiving the Prevnar-20 vaccine. She got the vaccine on 30Dec2025 and has never had side effects like this from a vaccine before. It's something new everyday that she's waking up. The injection site if still throbbing and she has a rash and scalloped tongue, massive dehydration. First she had regular chills and a reaction at the injection site. Then in 5 days she kept seeing red patches near the injection site then on Sunday she had a full rash on the inside of the same arm down the inside of the arm. Yesterday (05Jan2026) she woke up with the scalloped tongue and so much dehydration. She has follow up appointment with her doctor on Thursday (08Jan2026). Near the injection site it's pulsating again. Patient asked "Why is this going on so many days after the vaccine?". She had the shingles vaccine (manufacturer not specified) and no issues except the sore arm, had the tetanus vaccine (manufacturer not specified) and no issues except the sore arm. Patient stated she's getting very nervous as her tongue feels funny and she hopes it's not considered swelling, but she doesn't know. She read that there is also meningitis in the Prevnar-20 vaccine and wanted to know if this is true. She is calling about PCV 20. When attempting clarification on PCV 20, caller provided that it stated pneumococcal conjugate PCV 20 polysaccharide crm197 as the description listed from her doctors office. When querying manufacturer caller states she thinks its Pfizer because it states Pf at the end of the description listed from her doctors office. She got the vaccination at her pulmonologist office. Reported she has been getting crazy side effects. Her primary was not who gave her the vaccine. She will go see her primary doctor on Thursday (08Jan2026). The pulmonologist gave her the vaccine and is listed as the hcp on this report. Crazy side effects clarified as first she she got chills and fever and these were on the second day. Then on Sunday which she guesses was 5 days after, she had a rash on the inside of her left arm where she got the shot. She has pictures of it. When querying outcome of chills and fever, states they have recovered but she is feeling ill again. When querying outcome of rash, states rash is kind of faded now but now injection site is hurting her again. Treatment: she took ibuprofen. She was suffering massive dehydration and her tongue is scalloped and never had this before in her life. Dehydration and tongue being scalloped started yesterday. She has been drinking a lot of water but they are still there. It is messing with her speech. She believed she covered everything.
