vaccine was given outside of facility. R arm pain at site of injection. continues to have pan 4 months later.

tylenol, voltaren gel, cymbalta, pepcid, gabapentin, ibuprofen, myrbetriq, rosuvastatin, tadalafil, valacyclovir.

Allergies

strawberry, dust mites, penicillin

Lab data

ultrasound 9/25/25. MRI 1/5/26. EMG 1/2/26 all normal/no significant findings. Xray C spine with retrolisthesis, anterolisthesis, spondylosis
