Symptom narrative

Patient was administered Hep A instead of Pediarix. After administration I realized I had given the wrong vaccine. Provider notified at this time. I apologized to mom and explained what happen. Mom was okay with proceeding with the correct vaccine Pediarix. Patient was then given Pediarix. ADON notified and an incident report filed.
