Volunteer here with parent, [name withheld], to receive Hepatitis B booster following negative titer. Energix-B consent completed and reviewed, signed by volunteer and parent, VIS given and all questions answered. Energix-B 0.5ml given IM at 1007. Approximately a minute later Patient started jerking and slumped to left side, pale, unresponsive at first. Called for assistance and while [name withheld], RN coming Patient able to respond. Within 10-15 seconds Patient able to respond and answer questions. States feels nausea and is dizzy, affirms seeing spots. RN assisted Patient to put legs up on another chair and brought BP machine to room. BPs as follows: 1009 66/36 & P40 1012 85/46 & P60 1013 87/53 & P48 1028 94/54 & P49 During this time Patient drank juice, placed ice pack on neck to relieve sweating. C/o pain and tingling in R arm where injection given. Pulled up sleeve no redness, slight swelling palpated at and below site with diam of 4-5cm. Applied ice pack to area and [name withheld], parent, gave Patient two Ibuprofen 200mg tablets. Patient states no longer feeling dizzy or nausea. Face returned to normal complexion. Able to stand on own and walked to vaccine room to continue observation. Talking with mom and myself. All questions about what happened and what a vasovagal response is. Handout with written explanation given to parent by [name withheld], RN. 1045 91/58 & P53 Left at 1048 walking on own out door with parent. Lab slip provided for titer in 30 days. Suggested being sure to eat and drink prior to blood draw.
