Symptom narrative

As vaccine was being administered, patient moved arm, and the needle dislodged from the skin. Upon removal from the skin, the remainder of the vaccine sprayed out and sprayed the administering nurse in the face. Approximately 80% of the vaccine was administered to the patient.
