Patient was given Pediarix on 1/5/26 but should have received Boostrix and was also given the IPV vaccine which doubled up the polio doses. The State Health Department Representative (name withheld), and Pediarix vaccine manufacturer (GSK rep: withheld, Case ID withheld) were contacted as well as the patient's mom. [name withheld], RN talked to [name withheld] on the night of 1/5/26 and [name withheld] talked to the patient's mom on the 5th as well. The incident occurred after GSK vaccine reporting line was closed for the day so they were contacted at 8am on 1/6/26 by [name withheld]. Information collected from these sources said that based off CDC Guidelines, the patient will not need to be re-vaccinated for the TDAP vaccine and can remain on their normal vaccine schedule. Re-vaccination is also not required for the IPV vaccine based on NIH (NIH published this about receiving an extra dose)
