Symptom narrative

Patient was given a double dose of the HIB vaccine. Patient received a Pentacel and actHIB vaccine at the same time. Patient was supposed to get a daptacel and acthib only but the doctor order pentacel instead. The Medical Assistant gave what was ordered and did not recognize the error.
