Symptom narrative

Patient was mistakenly given a 3rd dose of flu shot. Nurse assumed he was getting it because the status of the room said flu shot and the reason for visit was "2nd flu" noted. No treatment needed. Parents were made aware. Corrective action reviewed with the nurse to always verify. the rights of medication and vaccine administration. (Right pt., medication, dose, route, time, documentation.)
