Symptom narrative

Patient 1 was in the same exam room as sibling. Both siblings needed vaccines. Medical Assistant brought vaccines in the room for both children. ProQuad and Quadracel was intended for patient's sibling, however, was given to patient 1 inadvertently.

Current illness

Acute cough and otitis media 1 month ago.
