PHN applied flu shot at 1615 followed by PPD application. Patient reported feeling lightheaded immediately following PPD administration. Patient became pale and diaphoretic. Brief syncopal episode lasting approximately 10-15 seconds witnessed by PHN. PHN requested for other PHN staff to call for Code RRT. Patient regained consciousness spontaneously, was able to verbalize name to PHN prior to CODE team arriving. Manual blood pressure at 1620 was 86/52 and was verbally reported to Code team. Patient's skin cool, pale and remained diaphoretic. Patient released to ED staff and transferred to ED for monitoring. Vital signs are within normal limits. Patient has a normal neurologic exam. States that lightheadedness has resolved. No other complaints at this time. I do feel the patient likely suffered from a vasovagal reaction to the needlestick. Immediate return precautions were discussed. Patient voiced understanding and was agreeable to plan for discharge. Patient discharged in stable condition.
