Symptom narrative

The patient presents today reporting a localized allergic reaction following a recent pneumonia vaccination. She states that couple of days after receiving the injection, she developed swelling at the injection site accompanied by erythema and pruritus. The patient describes the area as uncomfortable and itchy but denies severe pain. She reports no systemic symptoms such as fever, chills, shortness of breath, wheezing, facial or lip swelling, dizziness, or generalized rash. There is no history of prior severe allergic reactions to vaccines. The patient seeks evaluation to ensure the reaction is not serious and to receive guidance on management and monitoring. Patient was given SOLU-Medrol 125 mg Solution Injection to help with symptoms.
