administration of improperly stored vaccine; This spontaneous report was received from a medical assistant and refers to a 1-year-old patient of unknown gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 30-Oct-2025, the patient was vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) suspension for injection (lot #Y018582, expiration date: 03-Jul-2026), dose number 2, 0.5 mL administered as prophylaxis. The vaccine was administered after being improperly stored (Product storage error). The patient recovered from the event of administration of improperly stored vaccine on 30-Oct-2025. The action taken with Hepatitis A Vaccine, Inactivated (VAQTA) was not applicable. This is one of the multiple reports from the same reporter.
