Symptom narrative

Vaccine was administered to left deltoid. Following injection, a sizeable amount of liquid was observed leaking down the patient's arm. Upon further examination it was determined this was due to an incorrectly sized needle was used (5/8" and not 1"). Given the degree of fluid that was expelled from the injection site, it is questionable how much of the vacine dose was truly absorbed. Patient was offered another dose of the vaccine if she wanted to because of this, to which she accepted. There were no adverse effects or reaction noticed during the immunization process.
